This survey aims to understand the opinions, ideas and experiences of children, in their own words.  If needed, parents can help children understand the questions and the purpose of the survey, but please do not provide any answers on behalf of your child.  You can simply skip any questions that your child does not understand.  Thank you.

What is the survey about?

Children’s Services is asking for your help to make child care and other before and after school programs better for Toronto families.

Why is it important?

Your answers will help improve child care, before and after school programs, and other services for families like yours. 

How long will it take?

This survey will take 10-15 minutes.

What questions will be asked?

The questions will be about the types of programs you go to before and after school.

There are no right or wrong answers to the questions, and you can agree with some things a little, a lot, or somewhere in between (like picking foods you like or dislike).

If you choose to take this survey, some questions will ask information about you, like your age, but the survey does not ask for your name or keep track of who you are. 

You can choose 'I don’t want to answer' for any question or stop the survey at any time.

What if I don't understand a question?

You can skip any questions that you don't understand.  Please try not to guess.
